Useful Forms
Our office uses several forms that are absolutely imperative
for accurately collecting information to pursue our clients'
cases. Furthermore, there are several other forms used by
our office during the course of your claim. The following
are just a few of the forms that our office uses in each case
which we handle.

Client
Interview Sheet (ClientInterview.pdf)
This form is completed on each and every case that our office
handles. All of the information on this form is extremely
important in the handling of our clients' cases. It is necessary
that the Interview Sheet is completed in full, to the best
of our clients' abilities so that our office has accurate
information, not only about our clients' current case, but
about background information regarding medical history, injury
history and work history, that is vital during the course
of pursuing the clients' claims.
Statement
of Clients Rights (ClientsRights.pdf)
The Florida Bar requires this form to be given to each and
every client that signs a contract with our office.
Authorization
for Medical Information
(MedicalRelease.pdf)
This form is necessary for our office to obtain the medical
records of our clients. Frequently, additional authorizations
will need to be signed since many medical providers require
that the authorization be dated within six (6) months of the
request for information.

for General Information
(GeneralInformation.pdf)
Again, this form is necessary for our office to obtain
other types of records, including work records, school records,
military records, and other non-medical records.
Checklist
of Things to Do (Checklistofthingstodo.pdf)
This form provides information for our clients of things to do after
they are involved in an accident where they have sustained
injuries. Not only will the checklist ensure that our office
gets all the information needed from the clients, but it helps
our clients do things that will help in the pursuit of their
claims.
